All players are covered by insurance, comprehensive information available here. If a player is injured during a game or at training, this must be reported to the club within fourteen (14) days of the injury occurring. Failure to notify the Club of an injury within this time frame may jeopardise a player’s claim. Note this is only a report to notify the Club and MWFA that you have sustained an injury, this is NOT a claim. The information and links below will assist you should you wish to proceed with an insurance claim: -
  1. General Information Sheet - Sports Personal Injury Insurance – Provides guidance on how to claim and gives a summary of what is covered by your player insurance. Please read this carefully and note that items covered by Medicare cannot be claimed (see more information here) Note: - To claim some items such as physiotherapy, a referral from your doctor is required before commencing any treatment.
  2. Claim Form - This form needs to be lodged with QBE within 90 days of the injury. A club official will need to sign the form before it is submitted. Please contact the Club Registrar/s, Lou Caruso 0414 440 630 or Richard Hooker on 0425 416 190 to have this section completed. The MWFA must also sign before the form is submitted, please contact the office to ensure someone is available 9982 6228.
  3. Online Claim Forms - NOTE: Online claims submission is only available for injuries occurring on or after 1 January 2018.

  • Do not wait until your treatments have concluded before lodging your claim.
  • You can lodge your claim even if you have had no out of pocket expenses.
  • Government legislation prevents reimbursement of Medicare costs including the Medicare Gap.
